How this works

The PHQ-9 and GAD-7 are short questionnaires that doctors and NHS Talking Therapies use to screen for depression and anxiety and to track how someone is doing. Each answer scores 0–3.

PHQ-9: 0–4 minimal, 5–9 mild, 10–14 moderate, 15–19 moderately severe, 20–27 severe. GAD-7: 0–4 minimal, 5–9 mild, 10–14 moderate, 15–21 severe.

Good to know

  • A screening score is not a diagnosis — only a clinician can diagnose depression or an anxiety disorder.
  • A score of 10 or more on either scale is the usual point to talk to a GP or doctor.
  • Your answers are scored in your browser and never saved or sent anywhere.
